What should i put on my resume when im changing careers?

ok, about 5 years ago i was dental assisting in a dental office. I got out of that and got into childcare and am now a nanny. So for the last 4 years i have excellent childcare experience with excellent childcare references. Well, now i want to get back into dental assisting and employers would like for me to submit a resume. I have an excellent nanny resume but dont even have one for my jobs back when i was younger. I cant even remember exact dates of employment and if a new employer calls them for references they probably will not even know me. So my question is, should i just make up a "dental assisting" resume and not include my last 4 years of employment as a nanny seeing as how it would have nothing to do with the job i am applying for? Or include ALL employment no matter what job i am applying for. And what if i cant remember employment dates??????

I think I would include all emplyment so it does not look like you have employment gaps in your resume





The date issue is an interesting one - If you have your old tax records that might help you
